Quebec Population Health Survey will help give the Cree Health Board the big picture of health needs in the Cree Nation

Quebec Health Survey - Help us serve you better

The Quebec Population Health Survey will be held in Eeyou Istchee in four stages over the next year, from June to March, and will ask questions on physical and mental health, life style and other factors that can affect health. The survey will give a picture of the health and well-being of the people of Eeyou Istchee. 

You could be one of the 2000 Cree, aged 15 and over, selected at random for the survey. If so, you will receive a letter from the Institut de la statistique du Québec with info showing how you can take part. You can fill out the survey online, or over the phone, in Cree, English, or French. Any info you provide is anonymous. Your privacy is assured. 

If you prefer to take the survey over the phone in Cree, please call toll-free 1-844-330-6060. For English or French, call 1-866-390-5367. Someone will record your contact info and schedule the phone interview. Please have your file number when you call. Your file number (or numéro de dossier in French) is listed on the top left corner of the letter you received from the Institut de la statistique du Québec.

This survey will give the Cree Health Board and other Cree entities the big picture of health needs in the Cree Nation and help to build programs and services to make sure these needs are met. Your participation will help build the best health care services for the future of the Cree Nation.
